Generic:

• Generic Name: Fexofenadine Hydrochloride

Dosage Form:

• Form: Tablet
• Strength: 120 mg

Indications:

• Allergic Rhinitis: For the relief of symptoms associated with seasonal and perennial allergic rhinitis in adults and children 12 years of age and over. Symptoms include sneezing, rhinorrhea, lacrimation, itchy, red eyes, and itchy nose/palate/throat.
• Chronic Idiopathic Urticaria: For the relief of symptoms associated with chronic idiopathic urticaria in adults and children 12 years of age and older. Reduces signs and symptoms, including wheals and pruritus.

Pharmacology:

• Mechanism of Action: Fexofenadine is an H1 receptor antagonist that stabilizes the inactive form of the H1 receptor, preventing histamine activation and subsequent allergic symptoms. It does not cross the blood-brain barrier, minimizing central nervous system effects.

Dosage Guideline:

• Allergic Rhinitis:
  • Adults and Children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily. In cases of impaired renal function: 60 mg once daily.
  • Children 6 to 11 years: 30 mg twice daily or 60 mg once daily. In cases of impaired renal function: 30 mg once daily.
  • Children 2 to 11 years (Suspension): 30 mg (5 ml) twice daily. In cases of impaired renal function: 30 mg (5 ml) once daily.
• Chronic Idiopathic Urticaria:
  • Adults and Children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily. In cases of impaired renal function: 60 mg once daily.
  • Children 6 to 11 years: 30 mg twice daily or 60 mg once daily. In cases of impaired renal function: 30 mg once daily.
  • Children 6 months to less than 2 years (Suspension): 15 mg (2.5 ml) twice daily. In cases of impaired renal function: 15 mg (2.5 ml) once daily.
  • Children 2 to 11 years (Suspension): 30 mg (5 ml) twice daily. In cases of impaired renal function: 30 mg (5 ml) once daily.

• Administration: Orally

Interaction:

• Drug Interactions:
  • Increased plasma levels when coadministered with erythromycin or ketoconazole.
  • Antacids containing aluminium and magnesium hydroxide may reduce bioavailability. It is advised to separate antacid administration by 2 hours.

Contraindications:

• Hypersensitivity: Contraindicated in patients with known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients.

Side Effects:

• Common: Headache, drowsiness, dizziness, nausea.
• Uncommon: Fatigue.
• Rare: Hypersensitivity reactions (angioedema, chest tightness, dyspnoea), insomnia, nervousness, tachycardia, palpitations, diarrhea, rash, urticaria, pruritus.

Pregnancy & Lactation:

• Pregnancy: Category C. Should be used during pregnancy only if clearly necessary. Limited animal studies show no direct or indirect harmful effects.
• Lactation: Not recommended during breastfeeding as Fexofenadine may be present in breast milk.

Precautions & Warnings:

• Caution: Use with care in elderly patients and those with renal impairment. Cardiovascular patients should be cautious as antihistamines can cause tachycardia and palpitations.
• Driving and Machine Use: Unlikely to impair ability to drive or operate machinery. However, individual responses may vary.
• Renal and Hepatic Impairment: Adjust dosage as necessary for renal impairment. No substantial effect on hepatic function.
