Generic: Fexofenadine Hydrochloride


Indications:

  • Allergic Rhinitis: For relief of symptoms associated with seasonal and perennial allergic rhinitis, including sneezing, rhinorrhea, lacrimation, itchy, red eyes, and itchy nose/palate/throat.
  • Chronic Idiopathic Urticaria: For relief of symptoms including the reduction of wheals and pruritus.

Pharmacology:

  • Fexofenadine is an H1 histamine receptor inverse agonist. It stabilizes the inactive form of the H1 receptor, preventing its activation and subsequent allergic reactions. It does not cross the blood-brain barrier, minimizing central nervous system effects.

Dosage & Administration:

  • Allergic Rhinitis:
    • Adults and Children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily. For impaired renal function: 60 mg once daily.
    • Children 2 to 11 years: 30 mg or 5 ml twice daily. For impaired renal function: 30 mg or 5 ml once daily.
  • Chronic Idiopathic Urticaria:
    • Adults and Children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily. For impaired renal function: 60 mg once daily.
    • Children 6 months to less than 2 years: 15 mg or 2.5 ml twice daily. For impaired renal function: 15 mg or 2.5 ml once daily.
    • Children 2 to 11 years: 30 mg or 5 ml twice daily. For impaired renal function: 30 mg or 5 ml once daily.

Interaction:

  • Increased Plasma Levels: Coadministration with erythromycin or ketoconazole can increase fexofenadine levels by 2-3 times.
  • Antacids: Antacids containing aluminum and magnesium hydroxide may reduce fexofenadine bioavailability. Allow 2 hours between taking fexofenadine and these antacids.

Contraindications:

  • Hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients.

Side Effects:

  • Common: Headache, drowsiness, dizziness, nausea.
  • Uncommon: Fatigue.
  • Rare: Hypersensitivity reactions (angioedema, chest tightness), insomnia, nervousness, sleep disorders, tachycardia, palpitations, diarrhea, rash, urticaria, pruritus.

Pregnancy & Lactation:

  • Pregnancy: Limited data on safety. Should be used only if clearly necessary.
  • Lactation: Fexofenadine is found in human breast milk; not recommended for breastfeeding mothers.

Precautions & Warnings:

  • Use with care in elderly and patients with renal or hepatic impairment.
  • Antihistamines can cause tachycardia and palpitations in patients with cardiovascular disease.
  • No significant effect on driving or operating machinery, but individual response should be monitored.

Use in Special Populations:

  • Renal Impairment: Dose adjustments recommended for impaired renal function.
  • Hepatic Impairment: Moderate to severe hepatic disease does not substantially affect pharmacokinetics.
  • Elderly: Pharmacokinetics altered with increased bioavailability; monitor for adverse events.

Therapeutic Class:

  • Non-sedating antihistamines

Storage Conditions:

  • Store in a dry place away from light and heat. Keep out of the reach of children.
